Yep, your splines are not aligned between the disk and the transmission's driven gear. I'm assuming you used a tool to align the disk when you bolted down the cover. Put the trans in forth, and rotate the yoke until the splines align. Put tension on the transmission by pushing it towards the engine as you do it and you'll feel them align.
