I had the same issue 20 plus years ago. Number 4 plug was not tight. I tightened it and a week later same issue numerous 4 plug was loose. It was a bad plug. The ceramic was cracked and it was shorting out and heating the plug up. Like yours the gap was way too big due to a burnt post. Not sure what failed first and what caused what. No further issues in 20 years.
