Flat. That is certainly the case with the doors I have worked on. A good place to start is to also have the pin holes line up with each other - old way was to use a long rod.
I have been brought in to fit doors on a few antique vehicles - generally start with shims and at times end up the finish work with a 4x4 and tweaking.
Very often the issue is not the doors but with body mounts.
Not uncommon to take a few days to get doors to fit on a high end car
And yes to get door gaps absolutely even the edge of the door can get ground down and welded or built up with weld and metal finished.
