1st head gasket to blow - stock everything
replacement turbo - stock
Stock head - cracked at 46,000 miles - known for it, 9 mm injectore - replaced with a stock 7mm head.
Pump - stock, does have timing advance but that only relates to RPM, kicks in at 2300 rpm.
boost 23 lbs - Cummins manual states 35 max.
with the original stock turbo EGTs hit 1400 and were still climbing the one time it was tested under load 2900 RPM
With the new HX30/HX35 turbo (4BT) max EGT tempts are now 1100 - that is maxed out on the same hill, same load
Warped heads can happen because of the casting being green. The metal moves around from age and heat cycles. Good castings are aged and put through a few heat cycles before maching - not the case with Cummins Chinese castings.
Although the block deck and head deck are in spec I suspect the combination is no longer in spec and there are gases sneeking buy - even from idle. A pressure check in each cylinder may show a leak into another cylinder - maybe...
